Use this form to request a routine appointment such as a school physical.  Please do not use this form to request an appointment that you need in the next couple of days - this is only for appointments to be made in advance.  Please allow us 2 business days to respond to your request.  When you submit the form, you will see a confirmation page indicating what you have requested.  If any information is incorrect, then come back to this form and try again.  Once we receive your request, we will either email you with the date and time of the appointment or call you to set it up (in case we are unable to honor your request precisely).  Keep in mind that summer physical appointments fill up very quickly, so try to make your appointments early.
