12 Months

• MMR (Measles, Mumps, and Rubella)
• Prevnar (Pneumococcal conjugate)
• Varicella (Chickenpox)
• Flu Vaccine (if not previously given)

15 Months

• Pentacel (DPaP, Hib, IPV)
• Flu Vaccine (if not previously given)

18 Months

• Hepatitis A

2 Years

• Hepatitis A

3 Years

•  No immunizations

5 Years

• DTaP (Diphtheria, Tetanus, and acellular Pertussis)
• MMR (Measles, Mumps, and Rubella)
• IPV (inactivated Polio vaccine)
• Varicella (Chickenpox)

Thereafter:

• Tdap (Tetanus, Diphtheria, and acellular Pertussis booster) at 11-12 years, then a Td (Tetanus, Diphtheria) booster every 10 years.
• Menactra (Meningococcal meningitis) at 11-12 years
• HPV (Human Papillomavirus Virus) for girls at age 11-12. This is a series of 3 shots given over 6 months.
