- Flu Vaccine (Fall and Winter)
9 Months
- Anemia Check
- Flu Vaccine (if not previously given)
12 Months
- MMR (Measles, Mumps, and Rubella)
- Hib/Hep B combination (Haemophilus influenzae type B,
and Hepatitis B)
- Prevnar (Pneumococcal conjugate)
- Varicella (Chickenpox)
- Flu Vaccine (if not previously given)
15 Months
- DTaP (Diphtheria, Tetanus, and acellular Pertussis)
- Hep A (Hepatitis A)
- Flu Vaccine (if not previously given)
18 Months
2 Years
3 Years
4 Years
5 Years
- DTaP (Diphtheria, Tetanus, and acellular Pertussis)
- MMR (Measles, Mumps, and Rubella)
- IPV (inactivated Polio vaccine)
- Varicella (Chickenpox)
Thereafter:
- Tdap (Tetanus, Diphtheria, and acellular Pertussis booster) at 11-12 years, then
a Td (Tetanus, Diphtheria) booster every 10 years.
- Menactra (Meningococcal meningitis) at 11-12 years
- HPV (Human Papillomavirus Virus) for girls at age 11-12.
This is a series of 3 shots given over 6 months.
